General Description
- General Summary: Enhance the quality of patient management, maximize satisfaction, and promote cost effectiveness. Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client centered. Assist patients with utilization of their insurance benefits. Help clients achieve wellness and autonomy. Assess, plan, implement, monitor, and evaluate actions required to meet the client’s health and human services needed. Promote quality and cost effective interventions and outcomes.
- Duties: Identify patient’s available benefits with insurance company. Plan, organize, and individualize a plan of care based on mutual goals between patient and provider. Maintain database, and formulate statistical and narrative reports monthly. Serve as a point of contact, advocate, and resource for patient. Develop and organize a plan for patient compliance in yearly physical exams and testing. Communicate regularly with patients about their care and treatment plan. Identify and address patient’s risk factors and/or obstacles to care. Screen clients and/or population for healthcare needs. Accurately update and maintain patient cases. Provide patient education to help manage their healthcare needs. Perform related work as required in accordance to license, certification, and training when providing direct patient care. Must be cross trained to have the ability to efficiently and effectively work on scheduled Saturday shift. Provide compassionate patient care. Flexible hours. After hours may be necessary.
Work Experience Requirements
- Customer service (required)
- Two or more years experience as a LPN (preferred, not required)
- Two or more years experience in a case manager type role (preferred, not required)
- Basic computer skills (required)
- Two or more years experience in clinic setting (preferred, not required)
- Current LPN license (required)
- Current CPR certification (required)
- Excellent communication skills (required)
Education Requirements
- High school diploma or GED
- Successful completion of an accredited nursing program, LPN license must be current
