Job purpose

The Inspired Community Project (TICP) is seeking meticulous and compassionate Case Managers to manage the digital case files for children and families enrolled in our clinical daycare program. This critical role ensures the seamless organization, accuracy, and security of case files, which include prescriptions, Individualized Family Service Plans (IFSP), session notes, and progress reports. The Case Manager will act as a liaison between families, healthcare providers, and service coordinators, ensuring that all necessary documentation is current and accessible for Quality Assurance (QA) checks.

Aligned with TICP’s mission to promote equity, access, and support for underserved families, the Case Manager will help ensure that each child in the program receives consistent, high-quality care by maintaining efficient and organized records that support service delivery. This role is vital in creating a smooth, well-coordinated experience for both families and TICP’s staff.

Duties and responsibilities

  • Manage and maintain digital case files for all families, ensuring timely updates and organization of key documentation.
  • Collect and update family information, including medical prescriptions and IFSPs, in compliance with program guidelines and regulations.
  • Organize and track session notes, progress reports, and other required documents, ensuring they are accessible and properly stored.
  • Ensure all case files meet the standards required for Quality Assurance (QA) checks, conducting regular audits to guarantee completeness and accuracy.
  • Communicate with families to ensure that their information is up-to-date and that they have the necessary resources and documentation to support their child’s services.
  • Collaborate with healthcare providers, service coordinators, and internal teams to secure updated prescriptions, IFSPs, and other required documentation.
  • Support the Chief Programming Office & Family Advocate  in preparing case files for state and internal audits, ensuring compliance with all regulations and guidelines.
  • Stay informed of any changes in policies, documentation standards, or family service requirements to ensure files remain compliant.

Qualifications 

  • Highschool Diploma/GEO required.
  • 2+ years of experience in case management, preferably in early intervention, social services, or healthcare.
  • Strong organizational skills with a keen attention to detail and accuracy in managing digital files.
  • Excellent communication skills, both verbal and written, with the ability to engage effectively with families and service providers.
  • Proficiency in digital data collection
  • Ability to work independently and as part of a team, demonstrating initiative in problem-solving and task management.
  • Knowledge of early intervention regulations, IFSP documentation, and Medicaid compliance is a plus.
  • Bilingual Spanish is a plus

Position Details 

  • Position: Full-time position, 40 hours per week. Hybrid
  • Schedule: 7:30am - 3:30pm
  • Salary: $50,000 annually (Exempt)
  • Medical & 401K - immediately eligible upon hire

  • PTO Benefits: Total 7 weeks [36 days]
